Position Overview

The Housing Support Specialist plays a crucial role in delivering ongoing assistance and expertise through thorough evaluations, strategic planning, execution, and overall assessment of client requirements. The primary objective of this role is to improve client management quality and satisfaction, ensuring seamless care continuity and cost efficiency through integrated case management, review processes, and housing transition planning. This position is directly accountable to the Program Manager and is responsible for assessing the housing, health, financial, and emotional needs of program participants. Subsequently, the Housing Support Specialist will create and implement tailored service plans to address these needs.

Key Responsibilities

  • Deliver strength-based case management and service coordination to help clients secure and sustain permanent housing, including intake processes, housing strategies, case discussions, and follow-up plans.
  • Provide ongoing case management support for residents at 1, 2, 4, and 6 months post-discharge.
  • Maintain and update client records in compliance with data management systems, ensuring accuracy in documentation and goal tracking.
  • Prepare regular reports as mandated by organizational and funding bodies.
  • Develop individualized service plans for each client, monitoring progress and making necessary adjustments through case management sessions.
  • Assist clients in accessing financial support and healthcare services, advocating on their behalf as needed.
  • Implement and uphold program policies using evidence-based practices such as intensive case management and harm reduction strategies.
  • Acquire and maintain comprehensive knowledge of data management systems to fulfill data entry obligations.
  • Understand local and federal housing program regulations and assist clients with housing-related appointments and documentation.
  • Facilitate access to services that promote housing readiness, including financial literacy and tenant rights education.
  • Keep an updated directory of community resources for team utilization.
  • Collaborate closely with other departments and agencies to enhance participant outcomes.
  • Participate in weekly case management meetings to discuss service plan progress and emerging client needs.
  • Ensure compliance with confidentiality regulations regarding all client interactions.
  • Assist with front desk duties and support the Program Manager with events as needed.
  • Perform additional tasks as assigned by the management team.

Working Conditions

The role requires the ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or continuous basis. The candidate must be able to lift up to 25 lbs and navigate various physical tasks associated with the position. Vaccination against Covid-19 is a requirement for this role.

Qualifications

  • A Bachelor’s degree in Social Work is preferred, or an Associate’s degree in a related field, with at least one year of experience working with low-income or homeless populations.
  • A valid Class C California Driver's License is required.
  • Must successfully pass a criminal background check.
  • If working with children, a criminal background check is necessary in accordance with organizational policies.

Skills, Knowledge & Abilities

  • Understanding of the complexities surrounding homelessness.
  • Ability to engage with diverse populations in a compassionate and non-judgmental manner.
  • Basic knowledge of HIV/AIDS and bloodborne pathogens.
  • Familiarity with mental health disorders.
  • Proficiency in English; bilingual candidates (English/Spanish) are preferred.
  • Competence in Microsoft Office Suite, particularly Word and Excel.
  • CPR/First Aid certification is required.
