Job Summary

Nexus Health Systems Ltd is seeking a highly skilled Admissions Coordinator to join our team. As an Admissions Coordinator, you will play a critical role in converting referrals into actual admissions by managing all referrals from the time of referral to the time of actual admission.

Key Responsibilities
  • Effectively convert appropriate inpatient referrals into admissions within timeframes identified by referral sources.
  • Ensure that insurance verifications are completed for potential inpatient admissions to the facility.
  • Establish and maintain effective working relationships with referral sources.
  • Complete Intake Form accurately and completely, obtaining all pertinent financial and personal information on all referrals.
  • Complete benefits explanation documentation for patients and families.
  • Assure timely completion of screening for all referrals and relay timeframes to referral sources and all related parties throughout the admissions process.
  • Obtain and document clinical and financial acceptance from appropriate administrative staff for all referrals within established time frames and communicate to referral source and all related parties.
  • Secure a rate from the CEO or designee, if necessary, for commercial payors. Facilitate care plan and/or additional communication from facility case manager to the payor as necessary.
  • Coordinate all incoming transfers.
  • Document all interactions by date and time relative to each referral.
  • Complete and submit daily and monthly logs and reports.
  • Enter data into the patient accounting system as needed.
  • Attend all scheduled marketing meetings, as requested.
  • Complete follow-up phone calls on canceled referrals on a monthly/quarterly basis.
  • Liaison with the Texas Department of Assistive and Rehabilitative Services for potential inpatient/outpatient referrals.
  • Provide monthly referral, admission, and discharge tracking reports to the CEO and marketing team members.
  • Market facility programs and services to potential referral sources and conduct tours of the facility to potential referral sources and family members.
  • Participate in daily flash meetings with the CEO and/or key department managers or their designee to discuss referral activities.
  • Update daily referral activity and fax the intake board to the CEO and/or designees: per protocol.
  • Document all conversations and complete the intake checklist for each referral.
  • Demonstrate knowledge and awareness of Licensing regulations and third-party payer information.
  • Perform the above duties/responsibilities in inpatient and outpatient clinic settings.
  • Perform other duties as assigned.
Requirements
  • High school diploma or equivalent
  • Preferably some college courses
  • Experience in case management and insurance verification
  • Strong background in healthcare with knowledge of Medicare, Medicaid, and commercial insurance preferred
  • Microsoft efficiency
  • Medical terminology and demonstrated organizational skills necessary
