Position Overview
The Patient Care Coordinator (PCC) role at Houston Methodist is a registered nurse (RN) dedicated to the comprehensive management of patient care, focusing on care transitions and discharge planning for a specific patient demographic within designated units and service lines.

This role collaborates closely with physicians and the interprofessional healthcare team to ensure the delivery of compassionate, efficient, and high-quality care, aiming for optimal treatment outcomes.

The PCC shares accountability with the social worker for discharge planning and continuity of care, ensuring that admissions and ongoing stays are medically justified while effectively communicating clinical information to payors for reimbursement purposes.


Key Responsibilities

Effective Communication: Engages in proactive and constructive communication with all healthcare team members, reporting essential patient care and family information comprehensively and impartially; actively listens and values the contributions of others.

Collaboration: Works with the interprofessional healthcare team to enhance safety data and improve patient outcomes during care transitions. Utilizes structured communication methods with patients and families.

Patient Assessment: Conducts timely and thorough assessments of all patients. Participates in daily Care Coordination Rounds (CCR) to identify and communicate barriers to efficient patient throughput.

Discharge Planning: Facilitates discharge planning for assigned patients, collaborating with social workers and other healthcare team members, as well as patients and families, on complex discharge cases. Maintains ownership of the discharge planning process within assigned units.

Quality and Safety: Adjusts care based on ongoing evaluations of patient conditions, demonstrating clinical problem-solving and critical thinking. Documents assessments and interventions accurately and efficiently, focusing on discharge planning to meet departmental and hospital quality and safety targets.

Financial Management: Reviews medical necessity for admissions, continued stays, and resource utilization, ensuring compliance with nationally recognized screening guidelines. Manages patients in Observation Status, keeping physicians informed of timely disposition options to maximize patient benefits and hospital reimbursement.

Professional Development: Identifies areas for improvement based on evidence-based practices and initiates performance improvement projects. Actively seeks opportunities for self-development and engages in career discussions with management.

Qualifications

Education: Graduate of an accredited nursing program. A Bachelor's degree is preferred.

Experience: Minimum of three years of clinical nursing experience in a hospital setting; case management experience is preferred.

Licenses and Certifications: RN - Registered Nurse - Texas State Licensure and/or Compact State Licensure required within 60 days.

Skills and Abilities: Proficient in English communication, both written and verbal, with the ability to effectively interact with patients, families, and healthcare professionals. Knowledgeable in Medicare, Medicaid, and Managed Care requirements, as well as community resources and healthcare financial issues.

Work Environment: Business professional attire is required. On-call duties may be necessary during emergencies.
