Responsibilities
- Coordinate the admissions of hospice patients by conducting thorough assessments of their initial physical, psychosocial, and spiritual needs.
- Collaborate with the attending physician, medical director, and interdisciplinary team to establish the initial plan of care.
- Deliver skilled interventions and guidance to patients and caregivers to ensure optimal symptom management and comfort under physician direction.
- Act as a liaison among various disciplines and facilities during the admission process.
- Develop, prepare, and maintain accurate and timely individualized patient care progress records in accordance with policies.
- Submit precise documentation as per care center guidelines.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Communicate significant findings, issues, and changes in patient condition to the clinical manager, physician, and other members of the hospice care team.
- Liaise with physicians regarding patient needs and report any changes in condition; obtain physician orders as necessary.
- Fulfill on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform additional duties as required.

Qualifications
- Current RN license specific to the state(s) of employment.
- Minimum of one year of clinical RN experience.
- Current CPR certification.
