Job Summary

The successful candidate will be responsible for providing comprehensive nursing care to patients in their homes, as prescribed by physicians and in accordance with applicable laws and regulations. This role requires strong communication and interpersonal skills, as well as the ability to work independently and collaboratively as part of a healthcare team.

Key Responsibilities:
  • Conduct initial and ongoing assessments of patient needs, develop and implement individualized plans of care, and coordinate with other healthcare professionals to ensure seamless delivery of care.
  • Provide direct patient care, including administering medications, wound care, and other treatments as ordered by physicians.
  • Document all patient interactions, interventions, and outcomes in a timely and accurate manner, in compliance with regulatory requirements and agency policies.
  • Maintain current knowledge of state-specific guidelines, payer source requirements, and other relevant regulations governing home health care.
Requirements:
  • Current RN licensure in the state of Kentucky.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Benefits:
