Job Description: Home Health Nurse Specialist
At LHC Group, we're looking for a skilled Home Health Nurse Specialist to provide top-notch patient care in the home setting. This role offers a unique opportunity to make a meaningful impact on patients' lives while enjoying a great work-life balance.
Compensation: $70,000 - $90,000 per year
Duties and Responsibilities:
- Conduct comprehensive assessments, develop and implement individualized plans of care, and collaborate with physicians to ensure seamless care coordination.
- Evaluate patient progress, adjust treatment plans as needed, and communicate effectively with healthcare teams to ensure optimal outcomes.
- Manage case loads efficiently, prioritize tasks, and maintain accurate records to ensure regulatory compliance.
Qualifications:
- RN licensure in good standing, current CPR certification, and reliable transportation.
- Excellent communication and interpersonal skills, with ability to build strong relationships with patients, families, and interdisciplinary teams.
