Job Summary

The Nurse Care Manager - Home Health is responsible for coordinating and providing nursing care to patients in their homes. You will work closely with our healthcare team to develop and implement individualized plans of care, ensuring that each patient receives the highest level of care possible.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs and develop personalized plans of care
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination
  • Provide ongoing education and support to patients and their families
  • Maintain accurate and complete records of patient interactions and progress
Requirements

To be successful in this role, you must have:

  • A current RN licensure in the state of Kentucky
  • Current CPR certification
  • A reliable vehicle or access to public transportation
