About the Opportunity:
A dynamic and supportive environment is available for a Clinician to collaborate with a multidisciplinary team in Rochester NY. This role offers a chance to make a positive impact on the lives of youth and families.
Responsibilities:
- Contribute to the evaluation of client needs by utilizing therapeutic interventions to achieve positive outcomes.
- Collaborate with the clinical team to design, execute, assess, and modify individualized care plans, addressing each client's unique needs.
- Facilitate connections to programs and resources provided by the care team.
- Provide individual, group, and family therapy based on client requirements, partnering with informal supports, community organizations, and mental health professionals.
- Maintain accurate and timely documentation, including clinical evaluations, treatment plans, and progress reports.
- Serve as the primary point of contact for youth treatment plans.
- When necessary, provide transportation for youth to appointments, meetings, or other engagements.
Requirements:
- Master's degree in Social Work or Mental Health Counseling with a valid NYS Professional license.
- Minimum of 1+ years of relevant clinical experience working with youth and families facing mental health challenges.
- Thorough understanding of provided services, strong advocacy skills, and knowledge of payment processes.
- Valid driver's license and meet agency driving standards.
- Licensed or permit eligible upon hire.
- Additional requirements may apply, consistent with state professional licensing laws.
