Job Title: Community Care Coordinator

Salary: $55,000 - $75,000 per year

About the Role:

This position involves coordinating care for individuals and families in crisis, providing direct service coordination to ensure that needed supportive services and community resources are provided and accessed leading to an improved quality of life.

Key Responsibilities:

  • Conduct initial eligibility screenings, complete necessary intake and discharge paperwork, and review for appropriate utilization of services.
  • Develop individualized service plans for each client within ten working days of completing the assessment.
  • Re-evaluate service recipient status every thirty days and respond to changing needs and circumstances.
  • Maintain service recipient records and database for monthly, quarterly, and annual statistical reports.
  • Collaborate with clients, agency clinicians, and other departments to achieve goals and provide excellent customer service.

Requirements:

  • Bachelor's degree in social work or a related behavioral science area of study; or five years relevant experience.
  • Understanding of cultural differences and sensitivity to the implications of culture for service delivery.
