">We are seeking an experienced LVN/RN to act as a partner between our community and agency, seeking out referral opportunities and relationships, ensuring collaboration of care, coordinating discharge planning, and maintaining excellent communication.

- Develop relationships with physicians and clinical staff">
- Maintain effective communication with intake on next steps">
- Ensure patients receive the right service at the right time">
- Provide nursing care to patients who require it during idle time">
Requirements:
">To be successful in this role, you will possess:
">- ">
- An unrestricted LVN/RN license in the State of Texas">
- Minimum 2 years experience in hospice, medical surgical, critical care or oncology nursing">
- Excellent communication and organizational skills">
- A valid Texas driver's license and automobile insurance">
- CPR certification">
- TB status must be current">

Working Conditions:

">Prioritize Associate degree (Preferred) and minimum 1 year of nursing experience.
