**Job Description**

Devoted Care Hospice Inc is seeking a qualified Clinical Liaison to join our team. The Clinical Liaison will work closely with our healthcare professionals to provide exceptional care and support to our patients.

The estimated salary for this position is $65,000 - $85,000 per year, depending on experience and qualifications.

About the Position

The Clinical Liaison will be responsible for developing relationships with physicians and clinical staff to ensure seamless care transitions. This role requires a high level of organization and communication skills, as well as the ability to work independently and as part of a team.

Key Responsibilities

  • Act as a partner between the community and Devoted Care Hospice Inc by seeking out referral opportunities and relationships.
  • Coordinate discharge planning and maintain excellent communication with patients and families.
  • Develop relationships with physicians and clinical staff to ensure seamless care transitions.

Requirements

  • Unrestricted LVN/RN license in the State of Texas.
  • Minimum 2 years of experience in hospice, medical surgical, critical care or oncology nursing.
  • Excellent communication and organizational skills.
  • Hospice experience preferred.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid Texas driver's license and automobile insurance.
  • TB status must be current.
