Job DescriptionWe are seeking a skilled Pharmaceutical Care Coordinator to join our team at Option Care Health. As a key member of our infusion services team, you will play a crucial role in providing high-quality pharmacy services to patients across the United States.
Key Responsibilities- Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients
- Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient-specific information
- Ensure continuity of pharmaceutical care to and from the home and other patient-care settings
- Provide medication-use education to Option Care staff, other professionals, home care patients, and/or their caregivers
To be successful as a Pharmaceutical Care Coordinator, you must have:
- A graduate degree from an accredited School of Pharmacy
- A current, active Pharmacy license granted by the State Board of Pharmacy
- At least one year of experience in sterile IV admixture procedures or knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM)
We prefer candidates with experience in home care, hospital, or other clean room settings. Completion of an ASHP-accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy is also desirable.
CompensationThe estimated salary for this position is $95,208.92-$158,688.89, based on education, experience, knowledge, skills, and abilities. Benefits include 401k, dental insurance, disability insurance, health insurance, life insurance, paid time off, and vision insurance.
