At Inspired Home Care, we are seeking a skilled Clinical Care Coordinator to oversee patient care services and personnel. The ideal candidate will have a strong background in nursing and experience in home health care management.

About the Role

This is a full-time position that requires excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. The Clinical Care Coordinator will be responsible for coordinating patient care and integrating services provided by Agency staff.

Responsibilities
  • Coordinates patient care and integrates services provided by Agency staff
  • Ensures effective communication with physicians and other healthcare practitioners
  • Develops, implements, and updates individualized plans of care
  • Monitors quality and safety standards
  • Promotes compliance with regulations and industry standards
Requirements
  • Active RN license for the state of Texas
  • One year of home health care experience preferred, management experience preferred
  • Nursing skills, excellent interpersonal skills, effective communication, decision-making skills, and CPR certification (if required for visits)

The estimated salary for this role is $65,000 - $85,000 per year, depending on experience.
