Home Health RN Case Manager

As a Home Health RN Case Manager with the Good Samaritan Society, you will play a vital role in coordinating and integrating healthcare services for our clients. This position involves working with a variety of patients, including admissions, discharges, recertifications, and routine visits. We are seeking a skilled and compassionate nurse to join our team and provide high-quality care to our clients.
Responsibilities- Coordinate and integrate healthcare services for clients
- Develop and implement individualized care plans
- Provide education and support to clients and their families
- Collaborate with healthcare teams to ensure seamless care transitions
- Monitor and evaluate patient outcomes
- Bachelor's degree in nursing preferred
- Minimum of one to two years of professional nursing experience in a clinic care setting
- Currently holds an unencumbered RN license with the State Board of Nursing
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
