RN - Home Health Case Manager - FT Days - St. Peter Home Health

As a Home Health RN Case Manager, you'll play a vital role in integrating and developing service plans for our clients, while managing their care and providing guidance to staff.
Key Responsibilities:
- Coordinate and sequence client care from pre-admission to post-discharge
- Integrate health care services to enhance patient/resident flow and interdisciplinary communication
- Evaluate outcomes based on clinical, financial, and utilization data
- Work in structured and unstructured health care settings, utilizing independent nursing judgment
- Develop and implement care plans that address the unique needs of each client
Requirements:
- Bachelor's degree in nursing preferred
- Minimum of one to two years of professional nursing experience in a clinic care setting
- Currently holds an unencumbered RN license with the State Board of Nursing
Benefits:
- Competitive salary range: $34.00 - $48.00
- Flexible scheduling, with the option to create your own daily schedule
