Key Responsibilities- Develop and implement comprehensive care plans for clients, integrating health care services to meet their unique needs.
- Coordinate and facilitate communication among healthcare providers, family members, and other stakeholders to ensure seamless care transitions.
- Monitor and evaluate client outcomes, making adjustments to care plans as needed to optimize health and well-being.
- Collaborate with interdisciplinary teams to develop and implement evidence-based care practices.
- Provide education and support to clients and their families to promote self-management and independence.
To be successful in this role, you will need:
- A Bachelor's degree in nursing and a current RN license in good standing.
- At least one to two years of experience in a clinic care setting, preferably in home health or case management.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work independently and as part of a team, with a focus on delivering high-quality care and services.
