We are seeking a skilled and compassionate Home Health RN Case Manager to join our team at the Good Samaritan Society. As a key member of our care coordination team, you will play a vital role in ensuring the highest quality of care for our patients.
Key Responsibilities- Develop and implement comprehensive care plans that meet the unique needs of each patient
- Coordinate and sequence patient care to ensure seamless transitions and optimal outcomes
- Collaborate with interdisciplinary teams to promote early intervention and prevent hospital readmissions
- Monitor and evaluate patient responses to care, making adjustments as needed to ensure optimal outcomes
- Provide education and support to patients and their families to promote self-management and independence
- Bachelor's degree in nursing preferred
- Minimum of one to two years of professional nursing experience in a clinic care setting preferred
- Currently holds an unencumbered RN license with the State Board of Nursing
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
