Nurse Case Manager

We are seeking a highly skilled Registered Nurse RN Case Manager to join our team at Northeast Georgia Health System. As a key member of our healthcare team, you will be responsible for providing comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
Key Responsibilities:- Monitor patients on assigned units to ensure appropriate use of resources and interventions while managing patient length of stay based on working DRG/admitting diagnosis.
- Communicate with physicians, patients/families, and other disciplines the expected length of stay, along with patient progress towards discharge.
- Provide coordination and facilitation oversight of patient care to assure required interventions occur in proper sequence and processes occur in a timely manner without delays.
- Assess, coordinate, and facilitate patient discharge plans to ensure post-acute needs are arranged and secured prior to discharge.
- Maintain knowledge of reimbursement methodologies and general coverage guidelines for all levels of inpatient and outpatient care.
- Coordinate and communicate with Utilization Review Nurses on a daily basis to ensure patients are in the right status and level of care.
- Serve as a liaison to patients, families, physicians, nursing staff, and other disciplines to achieve optimal outcomes in the development of patient discharge plans.
- Actively support a customer service-oriented environment to continually enhance customer service.
- Licensure or other certifications: Licensed to practice as an RN in Georgia. CCM (Case Management Certification) or ACM (American Case Management Certification) required within 3 years of date of hire.
- Educational Requirements: Associates Degree. Graduate of an accredited school of nursing. Eligibility requirements for certification: 12 months full-time case management experience supervised by a certified case manager, or 24 months full-time case management experience not supervised by a certified case manager.
- Minimum Experience: Three (3) to five (5) years of experience in direct patient care and/or case management. Financial and discharge planning experience.
