Classification Title:
Clinical Case Manager I

Position Summary:
This role focuses on community-oriented education and case management services.

  • Manages an active caseload, delivering necessary interventions within the scope of expertise and credential limits; ensures the acquisition of additional services as required.
  • Gathers and shares comprehensive information about assigned clients to lay the groundwork for the Treatment Plan.
  • Collaborates with clients and their families, utilizing professional judgment to execute the Treatment Plan established by the team.
  • Aids in the formulation, execution, and modification of individualized treatment plans; ensures that provided services align with the Treatment Plan and tracks progress towards treatment objectives.
  • Offers customized oral hygiene and nutritional education to children and families, facilitating appropriate intervention linkages.
  • Supports families in enrolling their children in Medicaid and assists with necessary documentation.

Community Engagement and Navigation:
  • Engages with community systems to enhance linkage, referral, advocacy, and follow-up, focusing on achieving treatment objectives.
  • Develops engaging strategies with educational personnel to inform students and parents about community health initiatives.
  • Establishes and nurtures strong relationships with safety net organizations, maintaining knowledge of key contacts, eligibility criteria, referral processes, and follow-up outcomes.
  • Coordinates transportation for patients seeking services at relevant healthcare facilities.

Administrative Responsibilities:
  • Participates in meetings as needed and regularly consults with supervisors to exchange vital information and receive guidance.
  • Prepares and maintains client and program documentation in compliance with applicable standards and regulations.
  • Confirms patient Medicaid eligibility.
  • Engages in continuing education activities to remain knowledgeable in relevant areas.
  • Completes required training and ongoing certification.

Compensation:
$25/hr, based on education and experience

Minimum Qualifications:

Master's degree in a relevant field; or a bachelor's degree in a relevant field accompanied by two years of pertinent experience.


Preferred Qualifications:
  • CPR certification
  • Ability to travel to various locations as needed
  • Strong communication skills to build relationships with outreach program partners
  • Experience with pediatric populations
  • Demonstrates cultural sensitivity across diverse racial, ethnic, and socioeconomic groups
  • Previous experience within the Medicaid system
  • Familiarity with dental outreach programs
  • Knowledge of community resources, social services, and healthcare in the region
  • Ability to effectively demonstrate and communicate health education initiatives
  • Proficiency in Spanish or Haitian Creole
