TJJD - Senior Case Management Specialist - Institutional

GENERAL OVERVIEW
In this role, you will engage in advanced and supervisory case management activities, focusing on establishing and nurturing long-term relationships with assigned youth, their families, and various service providers to address medical, social, educational, and related needs. Responsibilities include conducting thorough case file evaluations, developing comprehensive case plans, facilitating family engagement, and delivering group services. You will set measurable goals and assess outcomes, ensuring effective coordination and evaluation of case management and program initiatives.
This position operates with minimal oversight, allowing for significant autonomy and independent decision-making. Flexibility in work hours, including potential overtime, varied shifts, and on-call duties, may be required.
TRAINING REQUIREMENTS
Newly appointed case managers or those returning after a significant break will be required to complete a training academy. Returning case managers within a specified timeframe will not need to undergo this training and will transition to full-time status upon re-employment.
KEY RESPONSIBILITIES
- Conduct initial assessments and case file reviews, ensuring all necessary documentation is complete.
- Develop and implement case plans tailored to the unique needs of each youth, utilizing appropriate strategies to promote progress and facilitate specialized referrals.
- Monitor caseloads to ensure effective transition planning.
- Lead and facilitate group sessions, planning for any necessary absences to maintain service continuity.
- Participate in ongoing professional development, including approved treatment programs and case management training.
- Provide individual counseling to youth and maintain regular family contact.
- Document services rendered, input assessment data, and maintain organized case files.
- Review and apply information from youth records to ensure health, safety, and security within the facility.
- Adhere to all relevant policies and procedures, maintaining accurate records and reports.
- Supervise the work of other staff as necessary.
- Report any observed disparities related to race, ethnicity, or gender.
- Perform additional duties as assigned.
- Ensure compliance with safety protocols, including reporting hazards and completing necessary documentation following incidents.
- Maintain regular attendance in accordance with agency policies.
QUALIFICATIONS
High School Diploma with ten years of relevant experience in case management or related fields, or a Bachelor's or Master's degree in a relevant discipline with varying years of experience.
KNOWLEDGE, SKILLS, AND ABILITIES
- Familiarity with community resources and child development principles.
- Proficiency in case management systems and practices.
- Strong skills in developing and monitoring treatment plans.
- Ability to respond effectively in emergencies and maintain discipline.
- Excellent organizational skills to manage workloads and prioritize tasks.
