The Rehabilitation Case Manager RN ensures high-quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes. This role provides, coordinates, and directs care specific to the needs of each rehabilitation patient.
Key Responsibilities- Provide program orientation to patients/families/caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient/family/caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Registered Nurse license in the state of FL or current active multistate license.
- Bachelor's Degree or Master's Degree required.
- Experience in outpatient/inpatient oncology, transplant, and/or cellular therapy required.
- 5 years of experience preferred.
- Certification in case management, nursing, or utilization preferred.
- Prior experience working within a Transplant and Cellular Therapy (TCT), Bone Marrow Transplant (BMT), or Blood and Bone Marrow transplant program is highly preferred.
