Clinical Case Manager

We are seeking a highly skilled Clinical Case Manager to join our team at InsideHigherEd. The successful candidate will be responsible for conducting needs assessments, developing individualized plans of care, and providing linkages to community resources for clients with HIV/AIDS.
Key Responsibilities- Conduct comprehensive needs assessments of clients with HIV/AIDS in outpatient and hospital settings to identify medical, social, and community resource needs.
- Develop and implement individualized plans of care, family support plans, and discharge plans to ensure timely access to and coordination of medical and psychosocial services.
- Provide education, linkage, and referral services to clients to address needs in collaboration with other Ryan White Network Providers.
- Advocate for clients' unmet needs and provide clinical intervention, including counseling and support, as needed.
- Develop therapeutic relationships with clients and maintain ongoing connections with clients and families during times of non-access to services.
- Provide linkages to agency, community, and government services and monitor referral completion, including state insurance continuation programs, ADAP, medication assistance programs, and benefits/health coverage programs.
- Document Ryan White service units and assist with monthly and quarterly reports for internal and external grant sources.
- Attend weekly clinical staffing and other divisional meetings to provide input on patient medical case management needs.
- Master's degree in an appropriate area, or a bachelor's degree in an appropriate area and two years of relevant experience.
- Experience with HIV/AIDS, hepatitis, and substance abuse prevention or service coordination with the target population is preferred.
