The Senior Community Care (SCC) program is a holistic, integrated care delivery initiative that coordinates clinical care for members in various settings. As an Advanced Practice Clinician (APC) within SCC, you will care for our highest-risk health plan members as part of a collaborative interdisciplinary team.

Key Responsibilities:

  • Perform thorough assessments of patients
  • Manage members through acute medical situations or exacerbations of chronic conditions, including diagnostics and interventions
  • Oversee members during short-term rehabilitation episodes
  • Ensure meticulous documentation, including ICD-10 coding
  • Collaborate effectively with the interdisciplinary care team
  • Engage in advanced illness and care planning discussions
  • Provide education and counseling to patients and their caregivers

Required Qualifications:

  • Graduation from an accredited Master of Science in Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in your state of residence, or the ability to obtain it by your start date
  • For Nurse Practitioners: Current certification through a national board, with specialization in one of the following:
    • Family Nurse Practitioner
    • Adult Nurse Practitioner
    • Gerontology Nurse Practitioner
    • Adult-Gerontology Acute Care Nurse Practitioner
  • For Physician Assistants: Certification through a national board, with graduation from an accredited Physician Assistant program
  • Current DEA licensure/prescriptive authority or the ability to obtain it post-hire, as per state regulations
  • Ability to establish a collaborative practice agreement, if required in your state
  • Capability to meet the physical demands of the role, including lifting and navigating various environments
  • Access to reliable transportation

Preferred Qualifications:

  • At least 1 year of clinical experience, preferably in a long-term care setting
  • Experience addressing the medical needs of patients with complex behavioral, social, and functional challenges
  • Knowledge of Geriatrics and Chronic Illness management
  • Familiarity with Advanced Illness and end-of-life discussions
  • Proficient in computer skills, including documentation in electronic medical records
