Position Overview:
Join our dedicated team as an Advanced Practice Provider specializing in Post-Acute Care. This role is ideal for professionals with a background in Family Medicine, Internal Medicine, or Geriatric Medicine.


Work Schedule:
Enjoy a full-time commitment with a flexible schedule, working four days a week, Monday through Friday. You will have the autonomy to set your own hours, with weekday call responsibilities limited to telephone consultations from 7 AM to 7 PM.
Core Responsibilities:
  • Deliver comprehensive primary care services in a post-acute environment for both short-term and long-term patients.
  • Manage chronic conditions, diagnose illnesses, and develop effective care plans.
  • Order and analyze laboratory results to inform treatment decisions.
  • Prescribe and oversee medication regimens.
  • Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
  • Provide education to patients and their families on managing chronic and age-related health issues.
  • Foster strong relationships with patients, their families, and facility staff to enhance treatment outcomes.
  • Encourage and promote healthy lifestyle modifications.

Qualifications:
  • New graduates are encouraged to apply.
  • Experience in primary care is advantageous; however, experience in post-acute care is not mandatory.
  • An active and unrestricted license as a Nurse Practitioner or Physician Assistant in Washington is required.
  • Possession of an active and unrestricted DEA registration is necessary.

Compensation and Benefits:
We offer a compensation model based on productivity (Fee for Service), allowing you to optimize your earning potential. The compensation range is between $93,000 and $114,000, accompanied by a comprehensive benefits package that includes:
