Reporting to: VP of Business Development
Position Summary: As a Community Outreach Associate, you will be instrumental in building meaningful connections within the community. This role focuses on engaging diverse groups, forming strategic partnerships, and enhancing True Care's visibility through innovative outreach initiatives. Your contributions will directly impact the growth and outreach success of our organization.
Key Responsibilities:
- Referral Generation: Actively pursue and secure referrals to expand True Care's client base.
- Partnership Development: Establish and maintain strong relationships with community organizations and stakeholders to enhance patient access to services.
- Strategic Relationship Building: Cultivate important relationships that emphasize long-term community engagement.
- Marketing Initiatives: Design and implement creative marketing strategies to boost True Care's visibility.
- Event Participation: Represent True Care at community events, demonstrating our commitment to service.
- CRM Management: Keep accurate records of outreach activities and communications in Salesforce.
- Additional Duties: Adapt to evolving organizational needs and perform other tasks as assigned.
Qualifications:
- Bachelor's degree in a relevant field.
- Experience in community outreach, marketing, or a related role, preferably in healthcare.
- Exceptional verbal and written communication skills.
- Strong organizational and time management abilities.
- Demonstrated leadership skills.
- Proficiency in Microsoft Office Suite and Salesforce.
Key Performance Indicators:
- Referral Generation Efficiency: Target of 15-20 new referrals per month.
- Community Engagement Impact: Engage with 4 community members or organizations monthly, aiming for a 25% increase in event participation.
- Partnership Development: Establish at least 3 new strategic partnerships each quarter.
- CRM Data Management: Maintain 100% accuracy in Salesforce entries.
- Start-of-Care Processes: Achieve a target of 7 successful start-of-care instances each month.
