Pathways Health and Community Support, LLC is on the lookout for a dedicated and visionary individual to become part of our team as a Client Care Coordinator.

Compensation: $22-$24 per hour with comprehensive benefits.

About Us:

We provide recovery-focused services tailored for adults (18 and older) eligible for Medi-Cal who are experiencing serious and persistent mental health challenges in the community.

Referrals are made through the local health care agency, ensuring that our services are well-coordinated, effective, and comprehensive.

Our center offers a wide range of services including Evaluation & Assessment, Psychiatric Treatment Planning, Follow-Up Care, Individual & Group Therapy, Crisis Intervention, Intensive Case Management, Medication Support, Family Education, Recreational Activities, Vocational Support, Community Resource Referrals, and regular Consumer Advisory Board Meetings.

Position Overview:

The Client Care Coordinator is responsible for outreach, information referral, care management, and delivering a comprehensive suite of mental health and rehabilitation services. This includes counseling, crisis intervention, skill development, and overseeing the assessment and treatment planning processes for each client.

Qualifications:
A Bachelor’s degree in human services, psychology, or a related field is preferred, along with at least one year of relevant experience in behavioral health.

Requirements:
A valid driver's license and auto insurance, current CPR certification, and proof of education are mandatory. Candidates should have a minimum of two years of prior case management experience and established connections within the community.
Strong interpersonal, organizational, and communication skills are essential.

Key Responsibilities:
- Conduct intake assessments and comprehensive evaluations of clients' situations, needs, and challenges, developing detailed care plans with client involvement.
- Coordinate services by assisting clients in executing their care plans, ensuring access to community resources, and monitoring the effectiveness of support services.
- Maintain regular contact with clients through home visits, providing personal support, consultation, and necessary interventions.
- Document daily activities, including service progress notes and billing forms, adhering to company and regulatory standards.
- Conduct outreach and provide information and referral services to individuals not requiring case management.
- Uphold HIPAA regulations, ethical standards, and professional boundaries with clients and staff.
- Offer temporary support to former clients as needed.
- Foster professional relationships with agencies and service providers.
- Participate in meetings and training sessions as required.
- Meet monthly productivity and documentation standards.
- Transport clients as necessary, utilizing personal vehicles.
- Handle on-call responsibilities as assigned.
- Perform additional duties as needed to support the department and organization.
