We are seeking a highly motivated and results-driven Outreach Executive to join our team at Oak Street Health. As a key member of our outreach team, you will be responsible for developing relationships with members of the community to engage Medicare seniors with our organization.

Key Responsibilities

* Develop and execute outreach strategies to engage Medicare seniors in our assigned territory
* Build and maintain relationships with community leaders, healthcare providers, and other stakeholders to promote our services
* Collaborate with our clinical team to ensure seamless patient onboarding and care
* Analyze data to identify trends and areas for improvement in our outreach efforts

Requirements

* 2+ years of experience in sales, marketing, or a related field
* Strong communication and interpersonal skills
* Ability to work in a fast-paced environment and meet deadlines
* Proficiency in Microsoft Office and CRM software
* Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary)
