Key Responsibilities:
  1. Care Coordination:

    Facilitate and promote quality care for adult patients in local and outreach clinics. Conduct systematic assessments of patient needs and develop tailored care plans. Identify and address barriers to care, coordinating necessary interventions.

  2. Interdisciplinary Collaboration:

    Communicate effectively with a multidisciplinary team, including specialists in neurology, orthopedics, and mental health, to ensure comprehensive patient assessments and care delivery.

  3. Patient Education:

    Provide education to patients and their families to support seamless transitions from pediatric to adult care. Develop educational materials based on patient needs and promote awareness of research opportunities.

  4. Clinical Support:

    Serve as a backup clinic nurse when necessary, maintaining accurate clinical databases and ensuring compliance with quality standards.
