Position Overview
Mental Health Case Coordinator As a vital member of the Assertive Community Treatment (ACT) team, the Case Coordinator will advocate for and collaborate with assigned adult clients facing severe mental health challenges. The ACT team is composed of diverse professionals, each focusing on specific client needs, including housing assistance, vocational support, therapeutic services, medical care, and substance use treatment.In this role, you will actively engage clients in the development and execution of personalized service plans, identifying their self-defined goals, service requirements, strengths, obstacles, support networks, and emotional triggers. Utilizing these plans, you will connect clients to essential community resources and provide direct support and training in essential life skills such as budgeting, grocery shopping, home maintenance, meal preparation, social interaction, coping mechanisms, and monitoring their psychiatric symptoms.
Regular home visits will be conducted to assess client progress and behaviors, collaborating with the ACT team to facilitate the achievement of client objectives. This position may require client transportation and includes a virtual or in-person on-call rotation every six weeks. For optimal client engagement, meetings will primarily occur within the local community.
