Medical Care Specialist Job Description at Charter Oak Communities

We are seeking a highly skilled Medical Care Specialist to join our team at Charter Oak Communities. As a critical member of our healthcare team, you will provide direct medical care to residents at Scofield Manor. Your responsibilities will include conducting assessments, planning and administering medication, and supervising care.

Job Responsibilities:
  • Develop and implement individualized care plans for residents.
  • Ensure accurate medication administration and maintain detailed records.
  • Provide training and development opportunities for staff members.
  • Collaborate with residents, families, staff, physicians, and other healthcare providers to ensure effective communication and high-quality care.
  • Adhere to all Local, State, Federal, and Charter Oak Communities' policies, procedures, standards, and codes.

Requirements:
  • State of Connecticut Licensed Practical Nurse (LPN) certification required.
  • Minimum four years LPN experience, preferably two in a managerial role.
  • Experience working with diverse populations and providing a range of services.
  • CPR certification mandatory.
