Job Summary

The Admissions Nurse - Geriatric Care Specialist is responsible for coordinating the admissions process for residents into the Sholom Community Alliance facility. This role involves receiving inquiries, conducting tours, and completing specific admission assessments. The successful candidate will work closely with the Director of Nursing to achieve budgeted census and collaborate with other departments to ensure seamless admissions, transfers, and discharges.

Key Responsibilities
  1. Admissions Coordination
    • Receive and respond to incoming inquiries for admission to the facility.
    • Conduct marketing tours for prospective residents and families.
    • Maintain accurate records of inquiry and lead tracking, follow-up, waiting list management, etc.
  2. Communication and Collaboration
    • Contact hospital social workers and/or nursing staff to gather accurate information on admissions.
    • Establish and maintain effective working relationships with referral sources and outside agencies.
  3. Admissions Assessment and Documentation
    • Perform admissions assessment responsibilities.
    • Prepare face sheets for medical charts and transcribe admission orders into the Electronic Medical Record (EMR).
  4. Resident Care and Support
    • Meet and welcome new residents and families upon arrival.
    • Assure residents are receiving appropriate care per their assessment and Care Plan.
    • Communicate with nursing staff and other departments to facilitate quality patient care.
  5. Compliance and Quality Assurance
    • Ensure compliance with Sholom established policies, procedures, and practices regarding the admissions process.
    • Support various other nursing responsibilities as needed.
Requirements
  • Graduate of an approved school of nursing and currently licensed as a LPN in the State of Minnesota.
  • Minimum of two years' experience working with geriatric patients and one year of care plan development experience preferred.
  • CPR Certification required.
Work Environment

The work environment is primarily indoors. This role requires the ability to work effectively in a team setting, communicate with various stakeholders, and maintain accurate records.
