Position: Admissions Assessment Nurse

Location: Minnesota

Work Schedule: 11:00 am - 7:00 pm, rotating weekends

Key Responsibilities:

  • Evaluate patients at the time of admission to determine appropriate care levels
  • Collaborate with internal personnel and a multidisciplinary team for effective care coordination
  • Manage admission documentation and uphold confidentiality standards

Required Qualifications:

  • Current Registered Nurse licensure in Minnesota
  • Bachelor's degree and prior experience in admissions preferred
  • Background in long-term care or acute care environments
  • Familiarity with MDS, Medicare, and various payer systems
