ROLE SUMMARY:

The Licensed Practical Nurse (LPN) operates under the supervision of the Registered Nurse (RN) and is accountable for delivering and overseeing high-quality clinical care and services to residents, in alignment with regulatory standards and established clinical best practices. Responsibilities encompass assisting with admission processes, training unlicensed personnel, administering medications, treatments, and personal care.

The LPN executes a variety of professional nursing tasks in the care of home care clients, adhering to state regulations and following established policies and procedures as required. This role will provide direct support to the RN, which may involve supervising, training, and monitoring unlicensed staff, conducting focused assessments, medication administration, auditing, charting, prescreening, admissions, and intakes, collaborating with physicians and insurance providers in transcribing and ordering medications under the RN's guidance, and communicating effectively with clients and their families. The position requires a high degree of independence, accuracy, and the ability to follow directions while taking initiative.

Comfort in supervising and training unlicensed staff is essential, as they will be responsible for medication administration under your guidance. Experience in Memory Care and familiarity with Point Click Care software is highly desirable.


KEY RESPONSIBILITIES:

  • Collaborates with the RN to assess clients' needs and changes in their condition, conducting focused assessments as necessary and reporting findings to the RN.
  • Ensures that unlicensed staff fulfill their responsibilities effectively and reports any issues to the RN.
  • Communicates changes in client conditions or other concerns to the RN promptly.
  • Initiates emergency protocols for clients and informs the RN accordingly.
  • Reviews and discusses client care plans developed by the RN and consults on treatment strategies.
  • Assists in implementing physician orders.
  • Maintains accurate charting and documentation practices.
  • Facilitates the initiation of services for new clients, transfers to other facilities, and discharges as directed by the RN Supervisor.
  • Assembles and operates necessary equipment for client care.
  • Orders, prepares, and administers medications.
  • Assists the RN in orienting, training, and mentoring new staff members.
  • Participates in required meetings, in-service training, and professional development opportunities.
  • Performs delegated tasks and other duties as assigned by the RN.
  • Supports the RN in evaluating the performance of unlicensed staff.
  • Coordinates and assists with the scheduling of unlicensed staff within established guidelines.

QUALIFICATIONS:

  • Current Licensed Practical Nurse licensure in the State of Minnesota.
  • Valid CPR certification.
  • Minimum of 1 year of experience in Long-Term Care is highly preferred.
  • Interest in gerontological nursing and rehabilitative clinical practices.
  • Trained Medication Aide (TMA) certification is a plus.
  • Experience with PointClickCare is preferred.
  • Knowledgeable about validation techniques and appropriate activities for clients with Dementia.
  • Must pass a criminal background check and not be listed on the OIG Exclusions List.
  • Must undergo TB screening.
