Role Responsibilities:

As a CARE Case Management Specialist, you will deliver extensive case management services to individuals and families requiring assistance. Your responsibilities will include:

  • Conducting thorough assessments of clients' needs and formulating personalized care plans.
  • Coordinating and overseeing the provision of services and resources to clients.
  • Maintaining precise and current case records and documentation.
  • Collaborating with other professionals and organizations to ensure clients receive optimal care.
  • Providing crisis intervention and support when necessary.

Qualifications:

Minimum Requirements:

  • Bachelor's degree in social work, psychology, or a related discipline.
  • A minimum of 2 years of experience in case management or a similar field.
  • Excellent communication, organizational, and problem-solving abilities.
  • Capability to work independently as well as collaboratively within a team.
  • Familiarity with community resources and services.

Preferred Qualifications:

  • Master's degree in social work, psychology, or a related discipline.
  • Experience working with diverse populations and/or in a non-profit environment.
  • Bilingual proficiency in English and Spanish.
  • Certification in case management or a related field.

Skills Required:

In this position, your strong communication and interpersonal skills will be vital in building relationships with clients and collaborating with other professionals. Your organizational and problem-solving capabilities will be crucial in developing and executing effective care plans. Proficiency in technology and software for maintaining accurate case records and documentation is also essential. Furthermore, your knowledge of community resources will be instrumental in connecting clients with the necessary support to achieve their objectives.
