Role Responsibilities:
As a Client Support Case Manager, you will provide comprehensive case management services to individuals and families requiring assistance. Your responsibilities will include:
- Conducting thorough assessments of clients' needs and creating personalized care plans.
- Coordinating and overseeing the delivery of services and resources to clients.
- Maintaining precise and current case records and documentation.
- Collaborating with other professionals and organizations to ensure clients receive optimal care.
- Offering crisis intervention and support as necessary.
Qualifications:
Minimum Requirements:
- Bachelor's degree in social work, psychology, or a related field.
- At least 2 years of experience in case management or a related area.
- Strong communication, organizational, and problem-solving abilities.
- Capability to work independently as well as collaboratively within a team.
- Familiarity with community resources and services.
Preferred Qualifications:
- Master's degree in social work, psychology, or a related field.
- Experience working with diverse populations and/or in a non-profit environment.
- Bilingual in English and Spanish.
- Certification in case management or a related field.
Essential Skills:
In this position, you will leverage your strong communication and interpersonal skills to build relationships with clients and collaborate with other professionals. Your organizational and problem-solving skills will be vital in crafting and executing effective care plans. Proficiency in technology and software for maintaining accurate case records and documentation is also required. Furthermore, your knowledge of community resources and services will be crucial in connecting clients with the necessary support to achieve their objectives.
