Position Overview

The Housing Navigator plays a crucial role within our agency's mission and vision. Reporting to the Program Manager of Housing, this position is responsible for delivering timely and effective housing navigation services to members authorized by Managed Care Plans. The Housing Navigator will facilitate housing deposits and tenancy support throughout the county, ensuring quality service delivery.

Key Responsibilities

  • Collaborate with team members to devise outreach strategies for engaging isolated consumers.
  • Commit to empowering consumer choice and fostering independence.
  • Build respectful relationships with consumers while maintaining professional boundaries.
  • Assist consumers in accessing healthcare resources, including primary and specialty care.
  • Support members in connecting with educational and employment services.
  • Enhance awareness of self-help programs and advocacy organizations that encourage recovery.
  • Encourage members to develop their skills and talents.
  • Participate in community presentations to introduce our services to potential consumers and partners.
  • Engage in regular staff meetings to contribute to program development and training.
  • Exhibit effective communication skills, ensuring cultural competence in interactions with a diverse member population.
  • Provide caregivers with relevant information about agency processes and support systems.
  • Conduct assessments to identify clients' housing needs and assist with housing applications.
  • Monitor clients' housing progress through regular meetings.
  • Identify resources for rent subsidies and affordable housing options.
  • Conduct inspections to ensure housing quality standards are met.
  • Assist clients in securing necessary household items for safety and accessibility.
  • Provide mediation services to resolve landlord-tenant issues.
  • Maintain accurate documentation of services in compliance with agency standards.

Qualifications

To excel in this role, candidates should possess a Bachelor's degree or equivalent experience in a social service-related field, along with knowledge of housing rights and resources. A valid California driver's license is required, along with the ability to navigate local community resources effectively.

Preferred Skills

  • Experience with Coordinated Entry Systems and local housing authorities.
  • Training in Motivational Interviewing and Trauma-Informed Care.
  • Ability to work independently and collaboratively within a team.
  • Strong communication skills, both written and verbal.
