Housing Support Case Coordinator

Are you passionate about transforming the lives of families experiencing homelessness? At Door of Hope, a faith-driven nonprofit organization, we are seeking a dedicated Case Manager to join our mission.
Role Responsibilities:As a Case Manager, your primary focus will be to assist families in navigating the challenges they encounter daily. This includes:
- Facilitating access to employment opportunities and sustainable income sources.
- Helping families build savings and secure affordable childcare.
- Guiding families in planning for their future through educational pursuits.
- Addressing legal and financial obstacles that hinder progress.
- Creating a safe and supportive environment for families escaping domestic violence.
Flexible scheduling options are available, including:
- 4/40, 9/80, or a standard 5-day work week with 8-hour shifts.
- Swing shifts that may include evening hours.
- Manage a caseload of up to ten families, ensuring personalized support.
- Engage in regular case conferences, team meetings, and training sessions.
- Orient new families to the program and coordinate their onboarding.
- Develop and oversee individualized case management plans, tracking progress through weekly meetings.
- Create and implement safety plans, including necessary transfers.
- Assist families in establishing realistic budgets and monitoring financial habits.
- Support families in accessing educational programs for long-term income growth.
- Build and maintain partnerships with relevant community agencies.
- Provide crisis intervention and advocacy during appointments with law enforcement and social services.
- Support the development and adherence to program guidelines.
- Act as a liaison between families and staff, ensuring clear communication.
- Document all interactions and maintain accurate records using the Apricot system.
- Oversee house management duties and provide coverage as needed.
- Perform additional tasks as required to further the mission of Door of Hope.
- A Bachelor's degree in Social Work or a related field is preferred.
- Bilingual candidates, particularly those fluent in Spanish, are highly desirable.
- 3-5 years of experience in case management is strongly preferred.
- Experience with domestic violence cases is a plus.
- Familiarity with resources available for families experiencing homelessness in Los Angeles County is preferred.
