Position Summary

As a Case Manager, you will deliver essential case management services to individuals with intellectual and developmental disabilities. Key responsibilities include:

  • Building positive relationships through a solution-focused approach with clients, their support teams, and external partners.
  • Exploring interests and growth opportunities for each individual.
  • Creating and overseeing a Person-Centered Plan that outlines necessary support and ensures ongoing service quality.
  • Meeting all state-mandated requirements for the Service Coordinator role.
  • Securing ongoing support services and assisting individuals with developmental disabilities.
Core Responsibilities
  1. Advocate for individuals, providing resources and education to promote self-determination.
  2. Gather documentation related to eligibility for services and assist with the application process.
  3. Develop and maintain Person-Centered Plans that reflect individual service needs.
  4. Identify and coordinate resources to achieve the goals outlined in each individual's plan.
  5. Monitor and evaluate the effectiveness of services provided.
Qualifications

Education:

  • A bachelor's degree in a human services field or a related area, or a non-human services degree with relevant experience.

Preferred Experience:

  • Experience working with individuals with developmental disabilities.

Other Requirements:

  • Successful completion of background checks.

Physical Requirements:

This role typically involves a standard office environment. Candidates should be able to remain stationary for extended periods and operate office equipment. Reasonable accommodations may be made for individuals with disabilities.
