Position SummaryAs a Case Manager, you will deliver essential case management services to individuals with intellectual and developmental disabilities. Key responsibilities include:
- Building positive relationships through a solution-focused approach with clients, their support teams, and external partners.
- Exploring interests and growth opportunities for each individual.
- Creating and overseeing a Person-Centered Plan that outlines necessary support and ensures ongoing service quality.
- Meeting all state-mandated requirements for the Service Coordinator role.
- Securing ongoing support services and assisting individuals with developmental disabilities.
- Advocate for individuals, providing resources and education to promote self-determination.
- Gather documentation related to eligibility for services and assist with the application process.
- Develop and maintain Person-Centered Plans that reflect individual service needs.
- Identify and coordinate resources to achieve the goals outlined in each individual's plan.
- Monitor and evaluate the effectiveness of services provided.
Education:
- A bachelor's degree in a human services field or a related area, or a non-human services degree with relevant experience.
Preferred Experience:
- Experience working with individuals with developmental disabilities.
Other Requirements:
- Successful completion of background checks.
Physical Requirements:
This role typically involves a standard office environment. Candidates should be able to remain stationary for extended periods and operate office equipment. Reasonable accommodations may be made for individuals with disabilities.
