Position Overview

The Housing Navigator operates within the agency's mission and values, reporting to the Program Manager of Housing. This role is pivotal in ensuring that members authorized by Managed Care Plans receive timely and effective transitional housing navigation services, including housing deposits and tenancy support throughout the county. The Housing Navigator collaborates with local homeless service systems, Managed Care Plans, and Enhanced Care Management providers to deliver quality services. A background in housing and homelessness, along with knowledge of HUD funding and regulations, is essential. Strong communication and organizational skills are necessary to thrive in a managed care environment.

Key Responsibilities

  • Collaborate with team members to devise outreach strategies for engaging isolated consumers.
  • Commit to empowering consumer choice and fostering independence.
  • Build respectful relationships with consumers while maintaining professional boundaries.
  • Assist consumers in accessing healthcare resources, including primary and specialty care.
  • Support members in connecting with educational and employment services.
  • Promote awareness of self-help programs and consumer advocacy organizations.
  • Encourage and nurture members' skills and talents.
  • Participate in community presentations to introduce the program to potential consumers and partners.
  • Engage in regular staff meetings to contribute to program development and training.
  • Maintain punctuality and consistent attendance.
  • Exhibit the agency's mission and core values in all interactions.
  • Communicate effectively with a diverse member population, fostering positive interactions.
  • Provide caregivers with information about agency processes and support systems.
  • Engage in continuous learning to enhance consumer engagement strategies.
  • Conduct assessments to determine clients' housing needs and assist with HMIS CES Surveys.
  • Meet with clients weekly to monitor housing progress.
  • Develop individualized housing support plans based on assessments.
  • Identify resources for rent subsidies and affordable housing options.
  • Conduct pre-inspections of housing units to ensure compliance with quality standards.
  • Assist clients in securing deposit assistance and necessary household goods.
  • Provide mediation services to resolve landlord-tenant issues.
  • Educate clients on their rights as tenants.
  • Maintain accurate documentation of services provided in accordance with agency standards.
  • Conduct health and safety home visits, including inspections.
  • Identify potential behaviors that may jeopardize housing stability and intervene early.
  • Collaborate with community resources to prevent eviction.
  • Assist with benefits advocacy and SSI application processes.

Additional Responsibilities

  • Perform other related duties as assigned to meet departmental needs.

Qualifications

To excel in this role, candidates must demonstrate the ability to fulfill essential duties satisfactorily. The following qualifications are required:

  • Bachelor's degree or equivalent experience in a social service-related field, or years of combined experience in case management and housing supportive services.
  • Knowledge of housing rights related to homelessness and low- to moderate-income housing.

Other Requirements

  • Valid California driver's license and insurable driving record, or demonstrated ability to use public transportation.
  • Extensive knowledge of local community resources is preferred.

Preferred Qualifications

  • Bachelor's degree or equivalent experience in a social service-related field.

Highly Desirable Qualifications

  • Familiarity with Coordinated Entry Systems, local Continuum of Care, and housing authorities.
  • Training in Motivational Interviewing, Trauma-Informed Care, and Harm Reduction.
  • Ability to work effectively with diverse communities.
  • Strong communication skills, both written and verbal.
  • Proactive and self-motivated with excellent prioritization skills.
