Three Rivers Healthcare Center, a proud member of the CommuniCare Family of Companies, is seeking a dedicated Full-Time Social Services Assistant / Designee to become an integral part of our team.

Position Type: Full-Time

Working Hours: 8:00 AM - 5:00 PM

MISSION STATEMENT:

The Social Services Assistant / Designee plays a crucial role in coordinating and implementing services that enhance the social and emotional well-being of each resident. This position ensures that care standards are upheld, delivering the highest quality of resident care at all times. The role requires collaboration with team members to achieve shared goals and to support the directives of the Social Services Director. In addition to focusing on quality care, the position involves managing available resources effectively and assisting colleagues in resource management.

KEY RESPONSIBILITIES:

  • Foster and maintain positive relationships with fellow caregivers to ensure their active participation in programs and services that support residents' psychosocial health.
  • Document and report any grievances or complaints to the Director of Social Services, ensuring timely communication.
  • Act as an advocate for residents, collaborating with care staff to ensure that all needs are met, including maintaining appropriate personal appearance and hygiene.
  • Provide valuable insights to the Director of Social Services to address resident concerns and enhance their care.
  • Enhance the quality of services and activities, expanding opportunities and choices available to residents.
  • Collaborate with the activities department to engage family members and guests in resident activities whenever feasible.
  • Uphold confidentiality regarding all resident and family information at all times.
  • Carry out general social services tasks as required.
  • Participate in ongoing training, educational programs, and orientation sessions to improve and maintain care quality.
  • Undertake additional duties as assigned or requested.
  • Adhere to established departmental policies, center protocols, and home office guidelines, including quality improvement initiatives and safety standards.

QUALIFICATIONS:

  • High school diploma or G.E.D. equivalent.
  • Prior experience in social services within a long-term care setting is preferred.
