  • Foster and maintain constructive relationships with direct care providers to support programs and services that enhance residents' psychosocial health.
  • Act as an advocate for residents' needs by collaborating effectively with all caregiving personnel.
  • Document and communicate any grievances, complaints, or changes in residents' conditions to the relevant staff members.
  • Work alongside the activities department to engage family members and guests in various activities.
  • Uphold the confidentiality of resident and family information at all times.
  • Engage in training and educational initiatives to improve the quality of care provided.
  • Adhere to established departmental policies and procedures to maintain quality care and safety standards.

Qualifications:

  • High school diploma or G.E.D. equivalent.
  • Prior experience in social services within a long-term care setting.
