Key Responsibilities
  • Conduct thorough social history assessments of patients to identify their needs, skills, interests, and limitations.
  • Develop and implement individualized treatment plans that promote recovery, symptom reduction, and improved social functioning.
  • Provide crisis management and family sessions to support patients and their families in understanding and coping with complex life situations.
  • Collaborate with multidisciplinary treatment teams to discuss patient assessments, treatment plans, and ongoing discharge plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Manage a caseload and maintain accurate patient chart documentation.
  • Facilitate education, process groups, support groups, and referrals as needed.
Requirements
  • Master's degree in a clinical field.
  • One or more years of experience working with patient populations preferred.
  • State licensure as an LSW, LPC, LPCC, or LISW.
  • CPR and de-escalation/restraint certification required.
