Position Overview

The Health System Pharmacist plays a crucial role in ensuring optimal pharmaceutical care within the Neurology ICU. This position involves direct patient engagement and collaboration with a multidisciplinary team to enhance medication management and therapeutic outcomes.

Key Responsibilities

A. Direct Patient Care

• Engage in patient care rounds within the Neurology ICU.
• Oversee acute management of adult patients in critical conditions.
• Evaluate medication profiles to ensure comprehensive pharmaceutical care for all co-existing health issues.
• Optimize medication therapies to minimize errors and enhance patient safety.
• Conduct therapeutic drug monitoring to assess appropriateness of medications, dosages, and potential interactions.
• Offer pharmacokinetic consultations as needed.
• Provide discharge counseling to patients when applicable.
• Share medication information with the interdisciplinary team.
• Collaborate with clinical staff to develop medication guidelines and protocols.
• Enhance Electronic Medical Record functionalities related to ICU patient care.
• Document all pharmacotherapy interventions accurately.

B. Educational Responsibilities

• Deliver patient-specific education as requested by medical and nursing staff.
• Design and implement educational lectures for pharmacy staff on critical care topics.
• Conduct in-service training for medical and nursing personnel on pharmacological issues.
• Develop guidelines and protocols pertinent to medicine and critical care.

C. Departmental Responsibilities

• Ensure safe and effective medication therapies in alignment with established protocols.
• Facilitate the creation and implementation of treatment guidelines and formulary changes.
• Coordinate the selection of alternative medications during shortages.
• Provide clinical consultations to healthcare practitioners.
• Support clinical initiatives within the pharmacy department.
• Monitor compliance with pharmacoeconomic programs regarding targeted medications.
• Recommend cost-effective therapeutic alternatives to medical staff.
• Assess drug therapy regimens for safety and appropriateness.
• Contribute to the pharmacy newsletter and participate in hospital education programs.
• Engage in quality improvement initiatives and maintain data on documented interventions.
• Foster collaborative relationships within the healthcare team.
• Mentor pharmacy students and facilitate their training in drug use evaluation.
• Participate in the Pharmacy and Therapeutics committee for formulary management.

Minimum Qualifications

• PharmD degree in Pharmacy is required.
• Valid New York State pharmacy license and registration.
• Current ACLS certification.
• Completion of PGY1 Pharmacy Residency or equivalent hospital experience.
• Critical Care PGY2 Pharmacy Residency or a minimum of three years of relevant practice experience is required.
• Board Certification in Critical Care Pharmacy is preferred.
