We are seeking a highly skilled Care Coordinator to join our team at Comprehensive Life Resources. As a Care Coordinator, you will play a critical role in coordinating and facilitating the WISe process for youth and families, providing advanced care coordination activities, and ensuring the delivery of high-quality services.
Key Responsibilities:
- Case Management: Provide case management services to children in foster care, ensuring their needs are met and they receive the necessary support to achieve their treatment goals.
- Collaboration and Coordination: Work closely with DCYF caseworkers, juvenile court, school personnel, and other team members to ensure seamless coordination of services and treatment.
- Pre-Placement and Placement Services: Arrange pre-placement visits, placement, and parent contacts to ensure a smooth transition for youth and families.
- CFT Meetings: Facilitate CFT meetings monthly to ensure all stakeholders are informed and engaged in the care coordination process.
- Skills Training and Support: Provide skills training, ILS activities, or other pro-social skills to youth to support their development and well-being.
- Consensus-Building: Utilize consensus-building techniques to meet the needs of the youth and family, ensuring all stakeholders are engaged and informed.
- Record Keeping: Maintain individual consumer records in a manner consistent with expectations.
- Crisis Intervention: Share in the provision of crisis intervention services to children and foster families.
- Therapeutic Services: Support and coordinate therapeutic services with foster parents and natural parents.
- Professional Standards: Ensure that ethical and professional standards of practice are maintained.
Qualifications:
- Bachelor's degree in a relevant field.
- One year of experience in a child-serving system, preferably in foster care and with adolescents.
- WA State credential/license.
- Effective written and verbal communication skills.
- Ability to transport consumers.
- Valid Washington State driver's license and proof of insurance.
