Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Care Coordinator in Pierce County. As a Care Coordinator, you will play a vital role in coordinating care services for our members, working closely with healthcare providers, and ensuring seamless transitions between care settings.

Key Responsibilities
  • Establish and maintain effective communication with members, healthcare providers, and caregivers to ensure comprehensive care coordination.
  • Conduct thorough assessments of member needs and develop individualized care plans to address their unique requirements.
  • Collaborate with healthcare providers to ensure timely and effective care delivery, including coordination of appointments, tests, and treatments.
  • Monitor member progress and adjust care plans as needed to ensure optimal health outcomes.
  • Provide education and support to members and their caregivers on health-related topics, including disease management and self-care.
  • Participate in quality improvement initiatives to enhance care coordination processes and outcomes.
Requirements
  • Current unrestricted Registered Nurse license in Washington.
  • 3+ years of RN experience, with a focus on case management or care coordination.
  • Experience working with electronic medical records and Microsoft Office.
  • Excellent communication and customer service skills.
  • Valid driver's license and ability to travel locally 75% of the time.
Preferred Qualifications
  • BSN degree.
  • Experience working in post-acute care settings, such as long-term care or hospice.
  • Case management experience as an RN.
  • Experience working with the geriatric population.
  • Familiarity with the long-term care business environment.
