Job Description

Comprehensive Life Resources is seeking a qualified Care Coordinator to join our WISe team in Tacoma, WA. As a member of our team, you will play a critical role in coordinating and facilitating the WISe process for individual youth and families, providing advanced care coordination activities, and case management services to children in foster care.

Responsibilities:

  • Provide case management services to children in specialized foster care, ensuring appropriate care and progress towards treatment goals.
  • Support and consult with foster parents, providing ongoing training and guidance on caring for children in placement.
  • Coordinate services with DCYF caseworkers, juvenile court, school personnel, and other team members to ensure seamless care coordination.
  • Participate in team staffing, arranging regular and emergency staffing as needed.
  • Facilitate CFT meetings monthly, providing skills training, ILS activities, and pro-social skills to youth.
  • Engage all CFT members, identifying their needs and priorities, and utilizing consensus-building techniques to meet the needs of the youth and family.
  • Work with the Youth Partner and/or Family Partner to identify family support, peer support, and community resources to assist the youth and family.
  • Share in the on-call status rotation and maintain individual consumer records in a manner consistent with expectations.
  • Provide crisis intervention services to children and foster families, ensuring ethical and professional standards of practice are maintained.

Qualifications:

  • Bachelor's degree in a relevant field.
  • One year of experience in a child-serving system, preferably with foster care and adolescent services.
  • WA State credential/license.
  • Effective written and verbal communication skills.
  • Ability to transport consumers.
  • Valid Washington State driver's license and proof of insurance.
