Clinical Case Manager I

Role Summary:
This position focuses on community-based education and case management.

  • Maintain an active caseload, providing necessary interventions within the scope of expertise and credentials, ensuring additional services are procured as required.
  • Gather and relay detailed information about assigned clients to the team, forming the basis for effective Treatment Plans.
  • Collaborate with clients and their families, utilizing professional judgment to execute the Treatment Plan determined by the team.
  • Assist in the creation, implementation, and adjustment of individualized treatment plans, ensuring services align with the Treatment Plan and monitoring progress towards goals.
  • Deliver customized oral hygiene and nutrition education to children and families, linking them to appropriate interventions.
  • Aid families in enrolling their children in Medicaid and completing necessary documentation.

Community Engagement and Navigation:
  • Consult and collaborate with community systems to enhance linkage, referral, advocacy, and follow-up aimed at achieving treatment objectives.
  • Develop engaging strategies with educational personnel to inform students and parents about dental outreach initiatives.
  • Establish and nurture relationships with safety net organizations, maintaining a comprehensive understanding of key contacts, eligibility criteria, referral processes, and follow-up outcomes.
  • Organize transportation for patients seeking services at designated healthcare facilities.

Administrative Responsibilities:
  • Attend relevant meetings and regularly meet with supervisors to exchange essential information and receive guidance.
  • Prepare and maintain client and program records in compliance with applicable standards, regulations, and grant requirements.
  • Verify patient Medicaid eligibility.
  • Engage in continuing education activities to remain knowledgeable in relevant areas of expertise.
  • Complete required training and maintain ongoing certification.

Compensation:
$25/hr, commensurate with education and experience.

Minimum Qualifications:

Master's degree in a relevant field; or a bachelor's degree in a relevant area with two years of pertinent experience.


Preferred Qualifications:
  • CPR certification
  • Ability to travel to various locations
  • Strong communication skills to foster partnerships within outreach programs
  • Experience with pediatric populations
  • Cultural sensitivity towards diverse racial, ethnic, and socioeconomic groups
  • Previous experience in the Medicaid system
  • Familiarity with dental outreach programs
  • Knowledge of community resources, social services, and healthcare
  • Ability to effectively demonstrate and communicate health education initiatives
  • Proficiency in Spanish or Haitian Creole
