POSITION SUMMARY

The role involves providing advocacy, resource linkage, referral services, and training in independent living skills for clients who require consistent support to thrive in their communities, particularly those eligible for Medicaid.

KEY RESPONSIBILITIES

  • Adhere to organizational policies and procedures, as well as relevant standards and regulations.
  • Foster a supportive environment for individuals seeking behavioral health services, ensuring a focus on recovery and trauma-informed care.
  • Build and maintain professional relationships with colleagues and supervisors, working effectively within a team.
  • Monitor client eligibility for various benefits and assist with applications for Social Security and Medicaid.
  • Keep accurate and updated records of client information, including contact details and financial data.
  • Engage with clients to assess their needs, provide face-to-face services, and follow up on service outcomes.
  • Coordinate client needs with available resources, advocating for services that prevent hospitalization.
  • Deliver intensive support to clients to aid in their outpatient recovery, tailoring service frequency based on individual assessments.
  • Assist clients in transitioning to lower levels of care as appropriate.
  • Participate in outreach efforts to connect with clients and professionals in inpatient settings.
  • Help clients access community resources to support independent living.
  • Facilitate the development of independent living and social skills among clients.
  • Support clients in creating personalized recovery plans that address their unique challenges and goals.
  • Maintain strict confidentiality regarding client and family information.
  • Provide a minimum of 85 hours of billable services monthly.
  • Utilize Collaborative/Therapeutic Documentation practices to ensure timely and accurate record-keeping.
  • Seek supervisory guidance for professional development and skill enhancement.
  • Collaborate with other professionals to share insights and resources, aiding in discharge planning from inpatient care.
  • Document services in accordance with agency policies, ensuring compliance with documentation timelines.
  • May require flexible working hours, including evenings or weekends, and the ability to provide transportation for clients.

QUALIFICATIONS

To excel in this position, candidates should possess a Bachelor’s degree in psychology, social work, or a related mental health field, or a Bachelor’s degree with a minimum of 36 behavioral health credits.
