POSITION SUMMARY
The role involves providing advocacy, linkage, referral, case management, and training in independent living skills to clients, particularly those eligible for Medicaid. This position requires regular interaction to support clients in maintaining their community presence.
KEY RESPONSIBILITIES
- Comply with agency policies and procedures, as well as relevant standards and regulations.
- Foster a supportive environment for individuals seeking behavioral health services, focusing on recovery and trauma-informed care.
- Build and maintain professional relationships with colleagues and supervisors, working effectively in team settings.
- Monitor client eligibility for benefits and assist with applications for Social Security and Medicaid.
- Keep accurate and updated records of client contact information and financial details.
- Assess client needs and deliver tailored services, ensuring follow-up on the effectiveness of the support provided.
- Coordinate resources for clients and their families, advocating to prevent unnecessary hospitalizations.
- Deliver intensive support to clients to facilitate outpatient recovery, adjusting service frequency based on individual assessments.
- Assist clients in transitioning between different levels of care and programs.
- Engage in outreach to connect with clients and professionals in inpatient settings to facilitate service referrals.
- Help clients access community resources essential for independent living.
- Support clients in developing essential life skills and community engagement activities.
- Guide clients in creating personalized recovery plans that address their unique needs and goals.
- Uphold strict confidentiality regarding client and family information.
- Provide a minimum of 85 hours of billable services monthly.
- Utilize Collaborative/Therapeutic Documentation practices to maintain accurate clinical records.
- Seek supervisory support for professional development and skill enhancement.
- Collaborate with other professionals to share insights and improve treatment approaches.
- Document services in compliance with agency standards, ensuring timely completion of records.
- May require flexible working hours, including evenings or weekends, and providing services in community settings.
- Transport clients using personal or agency vehicles as necessary.
QUALIFICATIONS
To excel in this role, candidates must demonstrate the ability to fulfill essential duties effectively. The following qualifications are required:
EDUCATION and EXPERIENCE
A Bachelor's degree in psychology, social work, or a related mental health field, or a Bachelor's degree with a minimum of 36 behavioral health credits.
