About Us: HOPE Community Services is a Certified Community Behavioral Health Center (CCBHC) dedicated to providing comprehensive mental health support to the South Oklahoma City Community. We are expanding our services and are in search of a compassionate Case Manager to enhance our Access and Intake Team, assisting individuals in accessing essential services.

Role Summary:

The Case Manager plays a crucial role in offering advocacy, resource linkage, referrals, and training in independent living skills to clients who require consistent engagement to maintain their community presence. This position primarily serves Medicaid-eligible individuals and ensures that new clients receive four essential services within the initial 45 days.

Key Responsibilities:

  • Comply with agency policies and procedures, as well as CARF and DMHSAS standards, ensuring adherence to federal and state confidentiality regulations.
  • Foster a supportive and recovery-oriented environment for individuals seeking behavioral health and co-occurring services, with a focus on trauma-informed care.
  • Build and maintain professional relationships with supervisors and colleagues, contributing effectively in team settings.
  • Monitor client eligibility and assist in applications for Social Security, Medicaid, and other potential benefits.
  • Keep accurate and current records of client contact information and financial details.
  • Establish rapport with clients, assess their needs, and deliver personalized face-to-face services while following up on service outcomes.
  • Assist clients in transitioning from initial services to ongoing programs tailored to their needs.
  • Coordinate client and family needs with available resources, providing advocacy as necessary.
  • Support clients in identifying and accessing community resources to promote independent living.
  • Facilitate individual and group sessions to develop clients' independent living and social skills, encouraging community engagement.
  • Help clients create personalized recovery plans that address their unique challenges and aspirations.
  • Maintain strict confidentiality regarding client and family information.
  • Provide a minimum of 85 hours of billable services to clients each month.
  • Utilize Collaborative/Therapeutic Documentation practices to ensure accurate clinical record-keeping.
  • Seek supervisory support for additional training and skill enhancement as needed.
  • Engage with other professionals to exchange insights on therapy, treatment, and new resources.
  • Document services in accordance with agency policies, ensuring timely completion of records.
  • Be available for evening or weekend hours as required.
  • Provide client transportation using personal or agency vehicles as necessary.
  • Perform additional related duties as assigned.

Qualifications:

To excel in this role, candidates must demonstrate the ability to fulfill essential duties effectively. Reasonable accommodations may be made for individuals with disabilities.

Education and Experience:

A Bachelor's degree in psychology, social work, or a related mental health field is required.

Certifications:

Must obtain and maintain DMHSAS CM Certification and complete required safety training and annual updates. First Aid and CPR certifications may also be necessary. A valid Oklahoma driver's license is required for client transportation.
